Welcome to Spirit Dermatology! We're excited to meet you. Here's what to bring to your first appointment.
What to Bring
Photo ID (driver's license or state-issued ID)
Insurance card (front and back)
List of current medications and dosages
List of known allergies
Relevant medical records or referral letter (if applicable)
Completed new patient forms (available for download soon)
New patient forms will be available for download soon so you can fill them out at home before your visit. In the meantime, please plan to arrive 10–15 minutes early to complete paperwork in the office.
What to Expect at Your First Visit
Your first visit is about building a relationship. Here's what a typical appointment looks like.
Check In
Arrive 10–15 minutes early to complete any remaining paperwork. Our front desk team will verify your insurance and get you settled.
Consultation
Denise will review your medical history and discuss your concerns, goals, and any symptoms. This is your time — ask questions freely.
Examination
A thorough skin evaluation tailored to your visit type — whether it's a full-body skin exam, a specific concern, or a cosmetic consultation.
Treatment Plan
You'll receive a personalized care plan with clear explanations, treatment options, and next steps. No jargon, no pressure — just honest guidance.
Most first visits take approximately 30–45 minutes.
Insurance Information
We accept most major insurance plans. Please call our office to verify your specific plan before your appointment.
A referral may be required by some insurance plans — check with your primary care provider.
Copays and deductibles are due at the time of service.
Cosmetic procedures are generally not covered by insurance and are paid out-of-pocket.
We will provide detailed receipts for any out-of-pocket procedures upon request.

Pre & Post Procedure Instructions
Following these general guidelines will help ensure the best possible results. Procedure-specific instructions will be provided at your consultation.
Before Your Procedure
Arrive with clean skin, free of makeup, lotions, or sunscreen on the area to be treated.
Avoid blood-thinning medications (aspirin, ibuprofen, fish oil) for 7 days prior, if medically safe to do so. Always consult your prescribing provider first.
Avoid retinoids, glycolic acid, and exfoliating products for 3–5 days before chemical peels or laser treatments.
Discontinue self-tanning products at least 1 week before laser procedures.
Stay well-hydrated and eat a light meal before your appointment.
Wear comfortable, loose-fitting clothing if a body area will be treated.
After Your Procedure
Apply sunscreen (SPF 30+) daily and avoid direct sun exposure on treated areas.
Keep the treated area clean and moisturized as directed.
Avoid picking, scratching, or touching treatment sites.
Avoid strenuous exercise, saunas, and hot tubs for 24–48 hours after most procedures.
Some redness, swelling, or mild bruising is normal and typically resolves within a few days.
Contact our office immediately if you experience signs of infection, excessive pain, or unexpected symptoms.
Important: These are general guidelines. You will receive personalized pre- and post-procedure instructions specific to your treatment plan. Always follow the instructions provided by your provider.
